Connected Care Program
At AIM Center, we believe in a holistic approach to mental health recovery. Our new Connected Care Services initiative ensures that individuals receive comprehensive support for both their mental and physical health, breaking down barriers to care and fostering lasting well-being. While AIM Center is a non-clinical facility, the Connected Care program specifically utilizes the model of integrated healthcare to be successful. By working strategically with health care providers, like River Valley, we are able to ensure care to individuals in need, regardless of their insurance status.
This program is available to both AIM Center members and non-members, helping to create a healthier, more connected community.

Services We Can Help With
Care Management
We assess individual needs, coordinate with health providers, offer education, and advocate for patients to ensure they receive the best possible care. Our team works tirelessly to bridge the gap between services and community needs.
Health Screenings
In partnership with River Valley Health, we assist with scheduling regular health screenings. Early detection and prevention of diseases are key to long-term health, and we make it easier for individuals to stay proactive in managing their well-being.
Behavioral Healthcare
Access to psychiatric services can be challenging, but AIM Center is committed to eliminating barriers to diagnoses and treatment. We provide essential support to those seeking behavioral healthcare, ensuring that no one falls through the cracks.
Community Connection
More than just a service provider, AIM Center fosters hope, community, and social change for individuals living with serious mental illness. Through our programs, we create opportunities for empowerment, connection and recovery.
